Controlled Substances:
- Controlled substances (medications such as benzodiazipines and stimulants – including but not limited to lorazepam (Ativan), alprazolam (Xanax), clonazepam (Klonopin), Adderall (amphetamine salts), Ritalin/Concerta (methylphenidate), Vyvanse (lisdexamfetamine) are NOT initiated during after-hours telehealth visits.
- If clinically appropriate, anyone prescribed or being considered for controlled substances/scheduled medications require an in-person visit during business hours for full face-to-face evaluation.
- If controlled substances / scheduled medications are continued, patients are required periodic in-person clinic visits in Clearfield, PA, during regular business hours (usually a minimum 2–3 in-person visits per year are required).
- Other policies and monitoring will apply if controlled substance prescribing is considered.
How it works:
- Request an appointment. Submit the online request and brief screening form.*
- We confirm fit and schedule. We review clinical fit, verify insurance (if using insurance), and offer the next available Friday evening slot.
- Complete intake paperwork. Required forms must be completed before the visit.
- Telehealth visit. Secure video visits are currently available Fridays 6–8 pm (by appointment only). Days/hours may change based on availability.
- Plan and follow-up. We create a treatment plan and schedule follow-ups. Same-day visits may be available occasionally, but this is not a crisis service.
- *Requesting or Scheduling an appointment does not guarantee medication prescribing, controlled substance prescribing, or ongoing care. Treatment recommendations are based on clinical judgment, safety, and appropriateness. We may recommend a different level of care, referral, or in-person follow-up when indicated.
Insurance and payment
- Commercial insurance and self-pay are accepted (provider is credentialed with most commercial insurance plans).
- After-hours appointments are not available for Medicaid or Medicare at this time.
- A valid credit card is required on file for all after-hours appointments. Patient responsibility (copays, deductibles, coinsurance, and any self-pay balances) is due at the time of service.Late cancellations and no-shows are subject to a fee, since after-hours appointments are limited and reserved in advance.

Q: When are after-hours appointments available?
A: Currently Fridays 6–8 pm, by appointment only. Days/hours may change based on availability.
Q: Can I book this if I already see you during weekday hours?
A: Yes. Existing patients can use Friday evenings for follow-ups when slots are available, as approved by provider and management.
Q: Can I use after-hours appointments for paperwork (forms/letters/disability/FMLA)?
A: After-hours visits are reserved for clinical care (evaluation, medication management, brief therapy support). Paperwork requests generally require a regular-hours appointment.
Q: Will you manage refills between visits?
Refills are handled as part of a planned follow-up schedule. If medication changes are needed, or if you missed an appointment, an appointment is required.
Q: Do you offer same-day Friday evening appointments?
A: Sometimes, if a slot opens. This is not for emergencies or crisis situations.
Q: If I’m new, what do I need to have ready for the first appointment?
A: Medication list, prior treatment history if available, and a pharmacy. Completing intake paperwork beforehand is required. We also require a valid photo ID.
Q: Can you diagnose and treat ADHD through telehealth?
A: Sometimes, when clinically appropriate. ADHD evaluation is completed through a structured or semi-structured clinical interview and review of symptoms, functioning, and history. Collateral information (prior records, rating scales, school/work history, or input from another source) may be required, and additional evaluation steps may be recommended. If a controlled medication is indicated, it is not initiated during after-hours telehealth—an in-person visit during regular business hours is required to start, with periodic in-person follow-ups thereafter.
Q: How do you handle vital signs or physical measurements with telehealth?
A: For telehealth visits, we may use recent measurements from your primary care office, a home blood pressure/heart rate log, pharmacy readings, or an in-office check when needed. If reliable vitals are required for safe prescribing, an in-person visit will be scheduled.
Q: What do I need for a telehealth visit?
A: A private location, a reliable internet connection, and a device with camera and microphone. If video is not possible, we will need to reschedule. You must be physically located in Pennsylvania and may be asked for verification. Telehealth is delivered by Zoom and so you may need to download or update the Zoom app.
Q: What ages do you see for after-hours telehealth?
A: Adults ages 18+ (edit this if you see 14–17). Pediatric care is scheduled during regular clinic hours.
Q: How often are follow-up visits required?
A: It depends on the condition and medications. Many patients are seen every 4–12 weeks. More frequent visits may be required during medication starts/changes or if symptoms are not stable.
Q: What is your cancellation/no-show policy for after-hours appointments?
A: After-hours slots are limited and reserved in advance. Late cancellations and no-shows are subject to a fee. If you need to reschedule due to an emergency, contact us as soon as possible—fees may be waived at our discretion.
Q: Can you prescribe on the first visit?
A: Sometimes. Prescribing depends on safety and having adequate clinical information. We may request records, collateral information, vitals, or labs before prescribing certain medications.
Q: Are any medications “off limits” for after-hours telehealth?
A: Some medications may not be appropriate to start or manage through an after-hours telehealth visit. For safety, controlled medications are not initiated during after-hours telehealth, and certain higher-risk medications may require labs, vitals, records, and/or an in-person visit during regular business hours. Prescribing is based on clinical judgment, safety, and medical necessity.
Q: What types of medications do you typically manage through telehealth?
A: Common outpatient psychiatric medications for anxiety, depression, ADHD, and related conditions may be managed through telehealth when clinically appropriate. Some treatments require additional monitoring (labs/vitals/AIMS) or in-person follow-up, which we will coordinate as needed.
Q: Are you a physician? What is a PMHNP? What is a nurse practitioner?
A: No. Brad is a Psychiatric-Mental Health Nurse Practitioner (PMHNP) and is also board certified as a Family Nurse Practitioner (FNP). A PMHNP is an advanced practice clinician trained to evaluate, diagnose, and treat mental health conditions, including prescribing medications when appropriate. In Pennsylvania, care is provided within applicable state laws and clinical standards.
Q: Can I use this service if I live anywhere in Pennsylvania?
A: Yes. You must be physically located in Pennsylvania at the time of the telehealth visit. Temporary residency (such as non-resident college student) may be considered, as long as you are physically located in Pennsylvania at the time of the telehealth visit.
